The predictive and prognostic value of risk factors in patients receiving hybrid coronary revascularization with postoperative pulmonary complications.

The mortality rate of coronary artery disease ranks first in developed countries, and coronary revascularization therapy is an important cornerstone of its treatment. The postoperative pulmonary complications (PPCs) in patients receiving one-stop hybrid coronary revascularization (HCR) aggravate the dysfunction of multiple organs such as the heart and lungs, therefore increasing mortality. However, the risk factors are still unclear. The objective of this study was to explore the risk factors of PPCs after HCR surgery. In this study, the perioperative data of 311 patients undergoing HCR surgery were reviewed. All patients were divided into two groups according to whether the PPCs occurred. The baseline information and surgery-related indicators in preoperative laboratory examination, intraoperative fluid management, and anesthesia management were compared between the two groups. Advanced age [odds ratio (OR): 1.065, 95% confidence interval (CI): 1.030-1.101, P<0.001], high body mass index (BMI; OR: 1.113, 95% CI: 1.011-1.225, P=0.02), history of percutaneous coronary intervention (PCI) surgery (OR: 2.831, 95% CI: 1.388-5.775, P=0.004), one-lung volume ventilation (OR: 3.804, 95% CI: 1.923-7.526, P<0.001), inhalation of high concentration oxygen (OR: 3.666, 95% CI: 1.719-7.815, P=0.001), the application of positive end-expiratory pressure (PEEP; OR: 2.567, 95% CI: 1.338-4.926, P=0.005), and long one-lung ventilation time (OR: 1.015, 95% CI: 1.006-1.023, P=0.001) may be risk factors for postoperative PPCs in patients undergoing one-stop coronary revascularization surgery. Using the above seven factors to jointly predict the risk of PPCs in patients undergoing one-stop coronary revascularization surgery, the receiver operating characteristic (ROC) curve showed an area under the curve (AUC) =0.873, 95% CI: 0.835-0.911, sensitivity: 84.81%, and specificity: 75.82%; the predictive model was shown to be effective. Patients undergoing HCR surgery with advanced age, high BMI, a history of PCI surgery, one-lung volume ventilation, inhalation of high concentration oxygen, use of PEEP, and prolonged single lung ventilation are more prone to PPCs.

Efficacy and safety of Tongxin formula after stent implantation for acute coronary syndrome: A multicenter, double-blind, placebo-controlled randomized trial

ObjectiveThe aim of this study is to comprehensively evaluate both the efficacy and safety profile of integrating the Tongxin formula with optimal medical therapy (OMT) for patients experiencing acute coronary syndromes subsequent to coronary stenting, over the course of one year. MethodsWe enrolled 150 patients diagnosed with acute coronary syndromes who had received stent placement within one month and exhibited a TCM syndrome characterized by Qi deficiency and blood stasis. This group comprised patients with unstable angina, non-ST-segment elevation myocardial infarction, and ST-segment elevation myocardial infarction. The participants were divided equally, allocating 75 to the Tongxin formula group and 75 to a placebo-controlled group. After undergoing percutaneous coronary intervention (PCI) surgery, both groups received conventional Western medical care, including dual antiplatelet therapy and lipid-lowering medications. The placebo-controlled group received a placebo, while the Tongxin formula group were administered Tongxin formula granules orally. Both study cohorts were monitored for a duration of 6 months. The primary endpoints included the occurrence of major adverse cardiovascular events and the rate of lumen diameter reduction post-treatment in both groups, with the Seattle Angina Scale serving as a secondary assessment tool. Safety evaluations encompassed the measurement of liver and kidney function, coagulation parameters, and other relevant indicators. ResultsThe rate of adverse cardiovascular events in the placebo-controlled group was 42.46 % within a year of surgery, whereas it was 16.90 % in the Tongxin formula group (P < 0.05). Comparing the Tongxin formula group to the placebo-controlled group, there was a decrease in the frequency of unstable angina and readmission due to cardiovascular events (P < 0.05). Coronary angiography performed 6 months after surgery revealed that the Tongxin formula group had considerably less lumen loss than the placebo-controlled group in a number of segments, including the entire segment, within the stent, at the proximal end, and at the distal end (P < 0.05). Six months after surgery, the Seattle angina score was higher in the Tongxin formula group than in the placebo-controlled group (P < 0.05). There were no significant changes in indicators such as liver and renal function as well as coagulation indexes in both groups within the first 12 months after surgery (P > 0.05). ConclusionTongxin formula has been shown to lower the occurrence of major adverse cardiovascular events, minimize narrowing of blood vessel lumen, enhance clinical symptoms, and enhance the quality of life of patients following PCI surgery, all while maintaining a good safety profile.

Interpretable machine learning for in-hospital mortality risk prediction in patients with ST-elevation myocardial infarction after percutaneous coronary interventions

Background and objectiveDespite the constant improvement of coronary heart disease (CHD) diagnostics and treatment methods it remains one of the main causes of death in most countries around the world. And myocardial infarction with ST segment elevation on the electrocardiogram (STEMI) still is one of the most dangerous clinical variants of CHD. This study aims to develop an explainable machine learning model for in-hospital mortality (IHM) risk prediction in STEMI patients after myocardial revascularization by percutaneous coronary intervention (PCI). MethodsA single-center observational retrospective study was conducted, enrolling 4677 electronic medical records of patients with STEMI after PCI, which were analyzed using statistical analysis and machine learning methods. A pool of potential IHM predictors was identified, and prognostic models were developed and validated based on multivariate logistic regression, random forest, and stochastic gradient boosting methods at two stages of hospital treatment: during the initial physicians examination in the emergency department and immediately after PCI surgery. To explain the IHM prognosis, threshold values of IHM risk factors were determined using 3 grid search methods for optimal cut-off points, calculating centroids and SHapley Additive exPlanations (SHAP). ResultsIHM prognostic models were developed using clinical and functional status data of STEMI patients during two stages of hospital treatment. The IHM prediction accuracy according to the first scenario was AUC = 0.85, and according to the second - AUC = 0.9. Predictors identified and validated in the models were converted into risk factors. Models whose parameters were risk factors demonstrated high forecast accuracy (AUC = 0.87), with the best model formed using the SHAP method. ConclusionsFor the forecast result interpretation risk factors obtained by categorizing continuous variables can be used by assessing the impact of the latter on the end point using the SHAP method.

ANXIETY LEVELS AND PERCUTANEOUS CARDIAC INTERVENTION (PCI) IN STEMI AND NSTEMI PATIENTS AT BANGIL HOSPITAL

According to the World Health Organization, coronary heart disease, including STEMI and NSTEMI, is the leading cause of noncommunicable disease-related mortality worldwide. First treatment for STEMI-NSTEMI patients was reperfusion with Percutaneous Coronary Intervention (PCI). Anxiety increases the difficulty of controlling cardiac disease in patients with a diagnosis of STEMI and NSTEMI who will undergo PCI surgery. This study employs a cross-sectional correlation analysis methodology. Patients with coronary heart disease diagnosed with STEMI and NSTEMI who are receiving treatment at Bangil Hospital constituted the population. Forty respondents were selected using a technique called purposive sampling. Percutaneous Coronary Intervention (PCI) is the independent variable. While apprehension levels are the dependent variable. At a Sig (2-tailed) value of 0.05, Ho was rejected and H1 was accepted, indicating that there was a correlation between preparation for surgery (Giving Informed Consent) Percutaneous Coronary Intervention and anxiety levels in STEMI - NSTEMI patients at Bangil Hospital. Consent was obtained prior to the decision to perform PCI. If the patient agrees to informed consent, the procedure will continue; if the patient refuses, the procedure will not be performed. The conclusion of this study is that Percutaneous Coronary Intervention (PCI) procedures are associated with a lower level of anxiety the better the preparation for surgery (providing informed consent) is.

Premature coronary artery disease in all-comers undergoing first PCI

Abstract Background The prevalence of coronary artery disease (CAD) increases with age, yet some individuals develop obstructive CAD at younger age. Purpose The present analysis aimed to assess differences in baseline risk profile and 3-year adverse event risk between patients undergoing percutaneous coronary intervention (PCI) for premature or non-premature CAD. Methods We pooled patient-level data of four randomized PCI trials in all-comers (TWENTE (clinicaltrials.gov: NCT01066650); DUTCH PEERS (NCT01331707); BIO-RESORT (NCT01674803); and BIONYX (NCT02508714)), who had no previous coronary revascularization (by PCI or coronary artery bypass surgery) or myocardial infarction (MI). Premature CAD was defined as CAD in men &amp;lt;50 years and in women &amp;lt;55 years, while non-premature CAD was classified as CAD in older patients. Main clinical endpoint was major adverse cardiac events (MACE), a composite of all-cause mortality, any MI, emergent coronary bypass surgery, or clinically indicated target lesion revascularization. Results Of all 6,171 PCI patients with first revascularizations, one out of seven (n=887, 14.4%) had premature CAD. These patients had lower rates of diabetes (10.6% vs. 16.6%, p&amp;lt;0.001), hypertension (32.7% vs. 50.7%, p&amp;lt;0.001), hypercholesterolemia (35.9% vs. 40.3%, p=0.012), and peripheral arterial disease (2.0% vs. 6.6%, p&amp;lt;0.001). Yet, they were more often overweight (76.2% vs. 69.8%, p&amp;lt;0.001), smoker (60.7% vs. 26.4%, p&amp;lt;0.001), and had more often a family history of CAD (59.8% vs. 44.5%, p&amp;lt;0.001). Patients with premature CAD suffered more often from acute ST-segment elevation MI and underwent less often treatment of calcified or bifurcated lesions (Table 1). At 3-year follow-up, the main endpoint MACE did not differ significantly between patients with premature and non-premature CAD (6.6% vs. 9.4%; adj.HR:0.86, 95%-CI:0.65-1.16, p=0.33; Figure 1). All-cause mortality was lower in patients with premature CAD (0.8% vs. 4.4%; adj.HR:0.23, 95%-CI:0.10-0.52, p&amp;lt;0.001). Yet, in patients with premature CAD the rate of repeated target vessel revascularization was higher than in patients with non-premature CAD (6.1% vs. 4.3%; adj.HR: 1.63, 95%-CI:1.18-2.26, p=0.003). In addition, their definite stent thrombosis risk was higher (1.0% vs. 0.5%; adj.HR:3.20, 95%-CI:1.39-7.38; p=0.006) than in patients with non-premature CAD. Conclusions One out of seven patients who were treated with PCI had premature CAD, and these patients had less complex risk profiles. Patients with premature CAD had higher risks for repeated revascularization and stent thrombosis, but the 3-year all-cause mortality risk was lower. In patients with premature CAD, long-term outcome after PCI might be improved by addressing modifiable risk factors such as smoking and overweight.Figure 1:Kaplan-Meier cumulative event

B-19 | Follow-Up Outcomes of Chronic Total Occlusion Percutaneous Coronary Intervention: Insights from the PROGRESS-CTO Registry

There is limited information on follow-up major adverse cardiac events (MACE) after chronic total occlusion (CTO) percutaneous coronary intervention (PCI). We examined the clinical and angiographic characteristics, as well as procedural and follow-up outcomes of patients who underwent CTO PCI at 38 US and non-US centers between 2012 and 2022 and had follow-up. Of the 3,488 patients with follow-up, 246 (7.1%) experienced MACE, including 73 deaths, 102 acute myocardial infarctions, and 122 target vessel revascularizations (with PCI or coronary artery bypass graft surgery [CABG]). The mean and median follow-up periods were 268±335 and 98 days, respectively. The incidence of MACE at 1, 2, and 3 years was 12%, 23%, and 27%, respectively. Patients with follow-up MACE were more likely to have comorbidities and had more complex lesions as indicated by higher Japanese CTO (J-CTO) scores (2.52 vs 2.33; p=0.026), and longer procedure (135 vs 110 min; p<0.001) and fluoroscopy (48.9 vs 40.0 min; p=0.006) times, higher contrast volume (250 vs 200 mL; p<0.001) and air kerma radiation dose (2.86 vs 2.20; p<0.001). The antegrade crossing strategy was less commonly the successful crossing strategy in patients with follow-up MACE (40.4%) compared with patients without follow-up MACE (58.3%; p<0.001). Cox regression analysis identified parameters associated with follow-up death, acute coronary syndrome, repeat target vessel PCI and target vessel coronary artery bypass graft surgery. Several parameters are associated with follow-up MACE after CTO PCI.

Application of two dimensional speckle tracking imaging in the follow up of patients with coronary heart disease treated with metalol

Purpose: To investigate the application of two-dimensional speckle tracking imaging (2D-STI) in the follow-up of patients with coronary heart disease (CHD) who were treated with metalol. Methods: A total of 100 patients with CHD (CHD group) who were treated with metalol and had undergone percutaneous coronary intervention (PCI) at Tangshan Works’ Hospital, Tangshan, China, and another 100 healthy people (control group) were enrolled in this study. Prior to and after 6 months of treatment, echocardiography was performed on patients in control and CHD groups, respectively, and the wall motion score was calculated. The 2D-STI was used to obtain systolic peak longitudinal strain value (SLs), global peak longitudinal strain (GSL) and early diastolic peak longitudinal strain rate (SrLe) in the early diastolic period of 6 wall basal segments, middle segments, and apical segments of the left ventricle. Results: The wall motion score index (WMSI) of CHD group was higher than that of the control group, while the WMSI of CHD group after treatment was significantly lower than the value before treatment, (p &lt; 0.05). In 2D-STI, the SLs, SrLe and GLS of patients after 6 months of treatment were significantly higher than those before treatment (p &lt; 0.05).Conclusion: Thus, 2D-STI can be used to quantify the motion of the chamber wall and the effect of PCI surgery, and it also provides a stable new index and technique for evaluating the effects of treatment of CHD.

Responsiveness and minimal clinically important difference of EQ-5D-5L in patients with coronary heart disease after percutaneous coronary intervention: A longitudinal study.

Although the five-level version of the EuroQol five-dimensional questionnaire (EQ-5D-5L) has been validated in various diseases, no empirical study has evaluated the responsiveness and minimal clinically important difference (MCID) of the instrument in patients with coronary heart disease (CHD), which limits the interpretability and clinical application of EQ-5D-5L. Therefore, this study aimed to determine the responsiveness and MCID of EQ-5D-5L in patients with CHD who underwent percutaneous coronary intervention (PCI) and identify the relationship between the MCID values and minimal detectable change (MDC). Patients with CHD were recruited for this longitudinal study at the Tianjin Medical University's General Hospital in China. At baseline and 4 weeks after PCI, participants completed the EQ-5D-5L and Seattle Angina Questionnaire (SAQ). Additionally, we used the effect size (ES) to assess the responsiveness of EQ-5D-5L. The anchor-based, distribution-based, and instrument-based methods were used in this study to calculate the MCID estimates. The MCID estimates to MDC ratios were computed at the individual and group levels at a 95% CI. Seventy-five patients with CHD completed the survey at both baseline and follow-up. The EQ-5D-5L health state utility (HSU) improved by 0.125 at follow-up compared with baseline. The ES of EQ-5D HSU was 0.850 in all patients and 1.152 in those who improved, indicating large responsiveness. The average (range) MCID value of the EQ-5D-5L HSU was 0.071 (0.052-0.098). These values can only be used to determine whether the change in scores were clinically meaningful at the group level. EQ-5D-5L has large responsiveness among CHD patients after undergoing PCI surgery. Future studies should focus on calculating the responsiveness and MCID for deterioration and examining the health changes at the individual level in CHD patients.

Correlation between coronary heart disease severity and subsequent chronic rhinosinusitis severity: A retrospective cohort study.

Coronary heart disease (CHD) is associated with the development of several diseases. This retrospective population-based cohort study investigated the association between CHD severity and subsequent chronic rhinosinusitis (CRS) of varying severity. We used data from Taiwan's National Health Insurance Research Database. CHD was categorized as severe if treated using a coronary artery bypass graft (CABG) and as mild if treated with percutaneous coronary intervention (PCI). The primary outcome of this study was the development of CRS or severe CRS treated using functional endoscopic sinus surgery. Cox proportional hazards regression was used to calculate adjusted hazard ratios (aHRs) and 95% confidence intervals (CIs) for CRS and severe CRS in different patient groups. We included 31,784 patients who received PCI surgery (the CHD-PCI group) and 15,892 patients who received CABG surgery (the CHD-CABG group). A total of 813 and 482 episodes of CRS occurred in the CHD-PCI and CHD-CABG groups, respectively, and 45 and 16 severe CRS events occurred in the CHD-PCI and CHD-CABG groups, respectively. Our multivariable analysis demonstrated that the incidence of CRS in the CHD-CABG group was significantly higher than that in the CHD-PCI group (aHR: 1.196, 95% CI: 1.064-1.280, P = 0.0402), but the two groups had similar incidence rates of severe CRS (aHR: 0.795, 95% CI: 0.456-1.388, P = 0.5534). Subgroup analyses revealed that the association between CHD severity and CRS development was more significant among men (P = 0.0016). In conclusion, we determined that severe CHD treated with CABG was associated with a higher incidence of subsequent CRS, and this association was more prominent among men.

Analysis of gut microbiota in patients with acute myocardial infarction by 16S rRNA sequencing.

An increasing number of studies have shown that gut microbiota are associated with human cardiovascular disease, but the characteristics of intestinal flora in patients with acute myocardial infarction (AMI) are still unclear. In this study, we aimed to investigate the difference of intestinal microflora between patients with AMI and healthy people, and to find the effect of percutaneous coronary intervention (PCI) on intestinal microflora. A total of 60 stool samples and 60 peripheral blood samples were collected from 20 previously diagnosed AMI patients and 20 healthy people serving as controls. Gut microbiota communities were analyzed via 16 ribosomal RNA-sequencing (16S rRNA). Gut microbiota-derived metabolites, trimethylamine N-oxide (TMAO) and short-chain fatty acids (SCFA), in the blood were detected using stable isotope dilution high-performance liquid chromatography with on line electrospray ionization tandem mass spectrometry (LC/MS/MS). The results showed that a distinct pattern of gut microbiota was observed in AMI patients compared to healthy controls. AMI patients had lower microbiological richness but no significant change in diversity. Bacteroidetes and Verrucomicobia showed an upward trend, whereas Proteobacteria showed a downward trend in AMI patients. During a longitudinal study to compare the changes in bacteria before and after treatment, we found routine cardiac admission therapy 1 week after PCI surgery had no effect on the microbial community structure in patients. There were significantly higher levels of plasma TMAO in AMI patients' microbiota than that in the control group. Contrarily, there was no obvious change in SCFA. The gut microbiota of patients with AMI differs from that of normal people, and the metabolic products of microflora are more abundant in the plasma of AMI than control cases. Microflora may act on the cardiovascular system through metabolites, and regulation of the microfloral structure may be used in the future treatment of cardiovascular diseases.

Abstract 13536: Cardiac Rehabilitation Enrollment Among Medicare Beneficiaries: Surgical Hospitals Play a Critical Role!

Introduction: Hospitalization for myocardial infarction (MI), percutaneous coronary intervention (PCI), or cardiac surgery [coronary artery bypass surgery (CABG) or valve surgery] represents a key opportunity to refer and recruit eligible patients to cardiac rehabilitation (CR.) However, little is known about how hospitals influence CR enrollment at the national level. Methods: Using Medicare fee-for-service claims from 2017, we identified all patients with a principal discharge diagnosis of MI, PCI, CABG, or valve surgery. We defined CR enrollment as attending ≥ 1 session within 90 days and calculated hospital-specific CR enrollment rates. To assure stability of our statistical results, we limited our sample to hospitals where cardiac surgery was performed, and then examined the association between patient demographics, procedures, comorbidities, and hospital characteristics on CR enrollment using mixed effects hierarchical logistic regression. We calculated the median odds ratio (MOR) to estimate the extent to which the hospital where a patient was cared for influenced their chance of enrolling in CR. Results: At 3,420 hospitals, we identified a total of 264,970 eligible patients (age 77 ± 8 yrs., 58% male, 85% white, 39% surgical), of which 25% attended CR. While a minority (38%) of hospitals performed cardiac surgery - these hospitals cared for the vast majority (92%) of all eligible patients. At these hospitals, the median CR enrollment rate was low (22%) and varied 10-fold across hospitals at the 10 th and 90 th percentiles (3%, 42%). After multivariable adjustment, the factors most associated with CR enrollment were having cardiac surgery or undergoing PCI (OR 5.8, 2.6 vs. MI respectively), advancing age (OR 0.77 per 5-year increase), low socioeconomic status (OR 0.33), smoking (OR 0.59), or living in the Midwest (OR 2.3 vs. South), p &lt;0.001 for all. The hospital MOR was 2.1, similar in effect size to PCI or Midwest location. Conclusions: In this large national study of older adults, participation rates in CR were low, and the hospital where patients receive their care was strongly associated with CR enrollment. As surgical hospitals care for the majority of all eligible patients, they appear to be the most logical place to focus improvement efforts.

Prior Coronary Artery Bypass Graft Surgery Impacts 30-day Quality of Life after Percutaneous Coronary Intervention: Evidence from the Victorian Cardiac Outcomes Registry (VCOR)

Quality of life following percutaneous coronary intervention (PCI) in patients with coronary artery bypass graft surgery (CABG) has been reported as lower than non-CABG patients, however previous reports pre-date modern developments in PCI and cardiac surgery. This study aimed to examine the 30-day QoL after PCI between patients with and without prior CABG using a contemporary dataset. A retrospective analysis of the Victorian Cardiac Outcomes Registry was undertaken. This study included 36,799 patients who completed the EQ-5D questionnaire that was used to assess the 30-day QoL and was compared between groups with and without prior CABG at baseline. Most of the participants were older than 65 years, more than half were male and had PCI due to acute coronary symptoms (ACS) and nearly 90% of patients received drug eluting stents. Compared to the ‘no prior CABG’ group, the ‘CABG’ group had a significantly higher rate of reporting a health problem (OR 1.30, 95% CI 1.10–1.53), presence of a problem in mobility (OR 1.42, 95% CI 1.15–1.75), personal care (OR 1.49, 95%CI 1.13–1.97) and usual activities (OR 1.39, 95%CI 1.15–1.68), pain/discomfort (OR 1.31, 95%CI 1.11–1.54), and anxiety/depression (OR 1.20, 95%CI 1.02–1.42). Despite modern developments in both PCI and CABG, our study showed a consistent negative association between prior CABG status and 30-day QoL following PCI. There is a need for better targeted cardiac rehabilitation in patients with prior CABG to address their greater relative risk of experiencing poor health.

Comparative characteristics of postoperative outcomes of different treatment strategies of patients with intermittent claudication in combination with coronary arteries disease.

Aim To compare in-hospital outcomes (severe cardiovascular complications, CVC) in patients with IIB stage chronic lower limb ischemia (CLLI) in combination with ischemic heart disease (IHD) in the following groups: stepwise percutaneous coronary intervention (PCI) and stenting and angioplasty of lower limb arteries (LLA) (group 1) and combination treatment, including PCI and open surgery on LLA (group 2).Material and methods Since 2019, the A.V. Vishnevsky National Medical Research Center of Surgery has performed a retrospective study that includes patients with stage IIB CLLI in combination with IHD. Patients were divided into 2 groups: group 1 (n=46), stepwise X-ray endovascular treatment (PCI and stenting and angioplasty of LLA); group 2 (n=46), stepwise combination treatment (PCI and open surgery on LLA). The endpoint included severe CVCs (death, acute myocardial infarction, acute cerebrovascular disease) and severe complications in the LLA area (stent thrombosis, repeated intervention on LLA, amputation).Results In 198 surgeries, none of 92 patients had severe CVC, and no fatal outcomes were observed. In group 2, there was one (2.1 %) severe complication on LLA during the early postoperative period, for which a successful additional intervention was performed.Conclusion Individualized approach to care of each patient with LLA pathology in combination with IHD helps avoiding severe CVCs at the hospital stage. It was shown that X-ray endovascular and combination treatments are safe and effective in the absence of fatal outcomes and acute disorders of coronary circulation at the hospital stage.

In recent years, medical simulators have been widely applied to a broad range of surgery training tasks. However, most of the existing surgery simulators can only provide limited immersive environments with a few pre-processed organ models, while ignoring the instant modeling of various personalized clinical cases, which brings substantive differences between training experiences and real surgery situations. To this end, we present a virtual reality (VR) based surgery simulation system for personalized percutaneous coronary intervention (PCI). The simulation system can directly take patient-specific clinical data as input and generate virtual 3D intervention scenarios. Specially, we introduce a fiber-based patient-specific cardiac dynamic model to simulate the nonlinear deformation among the multiple layers of the cardiac structure, which can well respect and correlate the atriums, ventricles and vessels, and thus gives rise to more effective visualization and interaction. Meanwhile, we design a tracking and haptic feedback hardware, which can enable users to manipulate physical intervention instruments and interact with virtual scenarios. We conduct quantitative analysis on deformation precision and modeling efficiency, and evaluate the simulation system based on the user studies from 16 cardiologists and 20 intervention trainees, comparing it to traditional desktop intervention simulators. The results confirm that our simulation system can provide a better user experience, and is a suitable platform for PCI surgery training and rehearsal.

Role of glucagon-like peptide-1 agonist in patients undergoing percutaneous coronary intervention or coronary artery bypass grafting: A meta-analysis

ImportanceGlucagon-like peptide-1 (GLP-1) protects against ischemia-reperfusion injury in patients with acute myocardial infarction (AMI). Controversy exists on the effects of GLP-1 on AMI patients undergoing percutaneous coronary intervention (PCI) and coronary artery bypass graft (CABG) surgery. Study objectiveWe aimed to investigate the cardioprotective effects of GLP-1 in AMI patients after PCI and CABG. DesignWe searched PubMed, Web of Science, EBSCO, Scopus, and the Cochrane Library for relevant randomized controlled trials (RCTs) up to June 2021, with no restriction on publication date. The following search terms are used: “percutaneous coronary intervention” or “coronary artery bypass grafting” or “myocardial infarction” and “glucagon-like peptide 1” or “exenatide” or “liraglutide”. Study selectionArticles were independently assessed by 2 reviewers. We included RCTs only that compared GLP-1 with control in AMI patients. Data extraction and synthesisContinuous data were pooled as mean differences (MDs), while dichotomous variables were pooled as odds ratios (ORs), with 95% confidence interval (CI), using R software (meta package) for windows. Subgroup analysis according to the intervention type and GLP-agents were conducted. We assessed the heterogeneity among RCTs using the Q statistic and I2 statistic. We also tested publication bias by funnel plot–based methods. The quality of each study was assessed with the Cochrane risk of bias tool. Main outcomes and measuresPrimary outcomes were changes of left ventricular ejection fraction (LVEF), myocardial infarct characteristics, salvage index. Secondary outcomes included major adverse cardiac events (MACE), gastrointestinal events, and hypoglycemia. ResultsNine RCTs (14 reports) including 1216 patients were included in this meta-analysis. At 3 months follow up, GLP-1 was associated with improved LVEF (MD = 2.81, 95% CI [0.69, 4.94]), infarct size in grams (MD = −5.71, 95% CI [−10.24, −1.18]), and salvage index (MD = 0.09, 95% CI [0.05, 0.14]). While, GLP-1 had less MACE rate than control (RR = 0.64, 95% CI [0.41, 0.99]), and higher gastrointestinal side effects (RR = 4.21, 95% CI [2.39, 7.41]). Conclusions and relevanceThis meta-analysis illustrated that GLP-1 was associated with better LVEF and reduced infarct size in patients with AMI undergoing PCI and CABG surgery, although the mechanism on how this agent provide this benefit is not clear. Diagnosis of Cardiac Rehabilitation after Percutaneous Coronary Intervention in Acute Myocardial Infarction Patients by Emission Computed Tomography Image Features under Filtered Back Projection Reconstruction Algorithm.

This study aimed to explore the application value of emission computed tomography (ECT) imaging technology based on filtered back projection reconstruction algorithm (FBP) in cardiac function examination after percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (AMI). Eighty patients with myocardial infarction diagnosed by medical history, electrocardiograph (ECG), and myocardial enzyme admitted to hospital from February 2018 to February 2019 were selected as the research objects. All patients underwent PCI seven days after the onset of myocardial infarction. ECT was performed for all patients before and after surgery. In addition, all ECT images were processed by the FBP reconstruction algorithm. On this basis, preoperative and postoperative cardiac surgery function and ischemia of the patients were diagnosed. Then, the diagnostic results were compared with the results of coronary angiography and echocardiogram. The results showed that all patients had a total of 541 segments before PCI surgery. ECT examination revealed 294 abnormal segments of the ventricular wall, with a total score of 585 points. A total of 100 segments were scored with 1 point, a total of 194 segments were scored with 2 points, and a total of 50 segments were scored with 3 points. After PCI, the number of abnormal segments was reduced to 58, with a total score of 193. There were 6 segments with a score of 1, 44 segments with a score of 2, and 5 segments with a score of 3. The left ventricular diastolic volume (EDV), left ventricular systolic volume (ESV), stroke volume (CO), and ejection fraction (EF) of the patients before the operation were 148 ± 16 mL, 77 ± 14.5 mL, 4.29 ± 0.37 L/min, and 41.9 ± 8%, respectively. The EDV, ESV, CO, and EF of the patients after surgery were 132 ± 16 mL, 62 ± 13 mL, 4.89 ± 0.71, and 53 ± 6%, respectively. Significant changes occurred in various systolic function parameters before and after surgery, P < 0.05. The standardized regression coefficients of the three groups were 0.32, 0.41, and 0.47, respectively, P < 0.05, which indicated that the greater the coronary artery stenosis rate, the higher the diagnostic coincidence rate of left anterior descending limb (LAD), left circumflex branch (LCX), and left coronary artery (RCA). The conformity of ECT imaging in the LCX group for diagnosis of myocardial ischemia was higher than that of UCG, P < 0.05. To sum up, the ECT technology based on the FBP reconstruction algorithm had a good application prospect in the diagnosis of cardiac function recovery in AMI patients after PCI.

Abstract Background Physicians always aim to improve their patients' health and consequently also improve community health. Continued medical education (CME) should be designed to support this goal and thus not only provide knowledge transfer, but also influence clinical decision-making and close performance gaps. However, the relative weight of CME in this process needs to be determined. Purpose Main purpose of the study was to analyse APT prescription rates and differences between subgroups of patients in a disease management programme (DMP) for coronary artery disease (CAD). Methods In a retrospective study we analysed prescription rates for APT in 254,932 CAD patients (male: 64.4%), treated in a total of 3,405 practices in 2019 in a DMP in the region of North Rhine, Germany, to delineate the potential for change by CME to further improve community health. Analyses were run for the whole study population stratified by sex as well as for subgroups of patients suffering from myocardial infarction / acute coronary syndrome (MI/ACS), or who have been treated with percutaneous coronary intervention (PCI) or bypass surgery. Subgroup differences were analysed in a logistic regression model. Results Patients mean age was 72.7±11.2 years (mean ± 1SD), mean duration of DMP participation was 7.2±4.7 years and mean cumulative number of DMP visits was 27±17. APT prescription rates were 85.0% in male and 78.8% in female CAD patients. In subgroups of male CAD patients APT prescription rates were between 89.7% and 92.8%, in the same subgroups of female CAD patients the corresponding rates were between 87.8% and 92.0%. Rates for a missing APT prescription per practice were between 0.0044% and 0.0062% for male and female CAD patients respectively. Rates for a missing APT prescription per practice and DMP visit were 0.0002% for both sexes. In a multivariate model significant positive predictors of APT prescription were male sex (OR=1.37, 95% CI: 1.34–1.40), age ≥76 yrs (OR=1.11, 95% CI: 1.08–1.14), participation in DMP ≥4 and &amp;lt;10 yrs (OR=1.19, 95% CI: 1.16–1.22), MI/ACS (OR=1.14, 95% CI: 1.08–1.20), PCI (OR=1.97, 95% CI: 1.88–2.06) and bypass surgery (OR=1.14, 95% CI: 1.08–1.21). Conclusion Our study demonstrates that a multifaceted intervention, like a DMP, can achieve high attainment rates for APT in CAD. To describe the benchmark for CME to further improve attainment rates, consideration of absolute numbers of eligible patients per practice or physician is probably more appropriate than expression of performance as percentage values. This is especially true if attainment rates show substantial variations between subgroups, if subgroups show substantial variation in size, if attainment rates are already in the order of magnitude of 80% or higher, and if there are disparities in the evidence base underlying treatment recommendations related to subgroups. Prevalence of coronary risk factors in contemporary practice among patients undergoing their first percutaneous coronary intervention: Implications for primary prevention.

BackgroundCigarette smoking, hypertension, dyslipidemia, diabetes, and obesity are conventional risk factors (RFs) for coronary artery disease (CAD). Population trends for these RFs have varied in recent decades. Consequently, the risk factor profile for patients presenting with a new diagnosis of CAD in contemporary practice remains unknown.ObjectivesTo examine the prevalence of RFs and their temporal trends among patients without a history of myocardial infarction or revascularization who underwent their first percutaneous coronary intervention (PCI).MethodsWe examined the prevalence and temporal trends of RFs among patients without a history of prior myocardial infarction, PCI, or coronary artery bypass graft surgery who underwent PCI at 47 non-federal hospitals in Michigan between 1/1/2010 and 3/31/2018.ResultsOf 69,571 men and 38,930 women in the study cohort, 95.5% of patients had 1 or more RFs and nearly half (55.2% of women and 48.7% of men) had ≥3 RFs. The gap in the mean age at the time of presentation between men and women narrowed as the number of RFs increased with a gap of 6 years among those with 2 RFs to <1 year among those with 5 RFs. Compared with patients without a current/recent history of smoking, those with a current/recent history of smoking presented a decade earlier (age 56.8 versus 66.9 years; p <0.0001). Compared with patients without obesity, patients with obesity presented 4.0 years earlier (age 61.4 years versus 65.4 years; p <0.0001).ConclusionsModifiable RFs are widely prevalent among patients undergoing their first PCI. Smoking and obesity are associated with an earlier age of presentation. Population-level interventions aimed at preventing obesity and smoking could significantly delay the onset of CAD and the need for PCI.

Fit, Female or Fifty - is cardiac rehabilitation fit for purpose? A systematic review and meta-analysis with meta-regression

Introduction Cardiac rehabilitation (CR) is a multi-disciplinary, evidence-based intervention, aimed to address modifiable risk factors for coronary artery disease. It is recommended worldwide for patients following myocardial infarction (MI), Percutaneous Coronary Intervention (PCI) and cardiac surgery. A growing body of evidence points towards a lack of uptake in CR in females and younger people. Purpose To examine the effectiveness of contemporary CR programs and assess whether they cater for all patients regardless of age, gender and prior level of fitness, via systematic review, meta-analysis and meta-regression. Methods MEDLINE was examined for studies involving exercise prescription or CR following MI, PCI and cardiac surgery from January 2010 to May 2020. RCTs and cohort studies of ≥10 patients were included for programs delivering phase II or III CR. Primary outcome measures were peakVO2max, 6-minute walk test (6MWT) and Metabolic Equivalent of Task (METs). Data were extracted using random effects meta-analysis. Epidemiological data were analysed for age, proportion of males to females and prior level of fitness. Baseline level of fitness was assessed by peakVO2max, 6MWT and METs values on entry into CR programs. Meta-regression was then used to determine change in fitness and the influence of age or gender. Results Thirty-three of the 713 studies (13 RCT, 20 cohort) were eligible and included in the review. Participants had a mean age of 60.0 years and 6/49 (12%) of study groups had a mean age &amp;lt;55 years. Male participants comprised 81.9% of those who completed CR and 41/58 (71%) participants were below average for cardiorespiratory fitness (CRF) when compared to normative values for those aged 60-69 years on entering CR. CRF improved in all study groups by the end of CR programs (mean improvement in peakVO2 3.3mL/kg/min, 6MWT 90.8m and METs 1.7). Meta-regression analysis showed that males were more likely to have an increase in 6MWT distance compared to females (mean difference 3.16m (95% CI 0.44-5.89). However, gender and age did not independently affect peakVO2max or METs. Conclusion CR following MI, PCI or cardiac surgery improved mean CRF in all study groups. While males were more likely to show an improvement in 6MWT there was no appreciable difference in effect in other outcomes after controlling for age or gender differences. Females, younger people and those of average or above CRF appear to be under-represented in data and attendance at cardiac rehabilitation. Given that CR outcomes are equal across gender and age, more effort should be made to encourage female and younger patients to attend. A ‘Precision Medicine’ model of exercise prescription may assist in this aim.
